Registered Nurse (RN) Case Manager As a Registered Nurse at Pathfinder Village, you will be joining a comprehensive team of RN's, LPN's, Clinical Office Assistants, and a Clinical Coordinator working in the rewarding field of developmental disabilities. You will work closely with our residents, their families, and clinical teams to assist and support them in making their healthcare decisions; provide staff support and education for assisting residents with their changing needs across their lifespan; and manage acute and routine healthcare needs. You will collaborate with the established relationships that we have with our local healthcare providers. In this position, you will provide telephone triage along with dedicated office and on-site hours coordinated by the Chief Clinical Officer. Qualifications: Bachelor's degree (BA or BSN) from an accredited four- year college or university, ORAn Associate's degree (ASN) from an accredited two-year private vocational school or community college with related experience Required Certifications: NYS Registered Professional Nurse License Valid NYS driver's license Current certification in CPR Annual salary: Starting at $80,000 | |
